Jacqueline Phyllis Wetzel Adams Black Medina passed away on June 21, 2011 after a difficult battle with colon cancer. Jackie was born on October 14, 1946 in Marysvale, Utah to Bert Wetzel and Rebecca Riding Wetzel. Her mother died when she was 5 years of age leaving 7 children. Jackie and her younger sister Pat were adopted by Orson and Juana Adams and moved to Carbondale Colorado were she lived until age 10. She then moved with her family to Alamosa, Colorado. When she was 17 she moved to St. George, Utah to live with her Grandma Riding where she met her future husband Leslie Black. Jackie and Leslie were married in St George on February 1, 1965. On May 15, 1990, after 25 years of marriage and 10 kids, her husband passed away. Two years later she met Donaldo E. Medina in Salt Lake City. They were married on December 3, 1992. They were married for 16 years before he passed away on March 6, 2009. Jackie is survived by her children: Dwain (Shela), Valerie Evans, Jason, Andrew (Jerrie), Jeremy (AnnMarie), Sabrina (John) Fryer, Alex, Donovan, and Jodi (Kyle) Nielson, stepchildren: Carol, Becky, Tina, Joe, Delores, Debbie, Rose, and Michael Ulibarri, numerous grandchildren and great grandchildren, siblings: Guy (Sharon) Wetzel, Matt (Virginia) Wetzel, Mauna Wilson, Fred Tull, Bert (Sherrie) Cuff, Bill (Mary Beth) Cuff, and Becky Barben, and many friends. She was preceded in death by her husband Leslie Black, husband Donaldo E Medina, parents Bert and Rebecca Wetzel, adopted parents Orson and Juana Adams, daughter Cassandra Black, step-son Donald Medina, brother Jim Wetzel, sister Pat Siminoe, sister-in-laws Deloy Wetzel, and Annette Tull. A viewing for Jackie will be held Friday June 24th from 6-8 pm in the Midvale 3rd Ward Chapel (7825 S Olympus St., Midvale UT). Funeral services will be held on June 25, 2011 at 11:00 am at the same location with a viewing starting one hour prior. Interment will be at the Hurricane City cemetery on June 27, 2011 at 1:00pm.
